A School-Based Health Center is an expanded school health program provided by a joint effort between the Jamestown Public School District and TRC Community Health Center.

TRC Community Health Center is recognized by the National Committee for Quality Assurance (NCQA) as a Level-III Patient Centered Medical Home (PCMH). A PCMH is a health care setting that emphasizes partnerships between individual patients and their personal physicians.
We Support Our Community
TRC Community Health Center is a NHSC-approved site that provides primary health services. NHSC supports health centers in rural and urban communities. The National Health Service Corps (NHSC) supports more than 18,000 primary care medical, dental, and behavioral health providers through scholarships and loan repayment programs.
