Why A School-Based Health Center?

Instead of taking time off of work and your child missing pivotal class time, enroll them in our SBHC. A school-based health center is a safe, easily-accessible medical office on a school campus where students can go for comprehensive, preventative, and primary health and dental care services. Children cannot reach full education potential unless they are physically and mentally healthy.

How Does The School-Based Health Center Work?

Our SBHC doesn’t take the place of your family’s physician or dentist, but instead works as an additional service to enhance the care of the student in a convenient location. Your physician will always be informed of any treatment or problems. If you don’t have a physician or insurance, we will work with you to determine what options are available. Nobody will be turned away because of an inability to pay and parents will always be contacted prior to most treatments.

  • Dental Cleanings
  • Ability to Email X-Rays to Family Dentist

When the school is not in session, PLEASE call your primary care provider.  If you do not have a primary care provider, please call 716-661-1447.

The School Based Health Center at Jamestown High School also provides mental health services for students who are in need of these professional-based services. Today’s student population can face a lot of pressures and emotional stress not only from the demands of school but from peer pressure. It’s important to seek help and medical attention when needed. The School Based Health Center at Jamestown High School provides these important services on site.

Patient Portal

Community Inclusion, Inc. has adopted a program called AthenaHealth. This is an online patient portal that enables patients to take a proactive role in managing their care at their own convenience. You can request refills, talk to your provider, obtain your test results, and get a real-time response to the status of a refill!  Please click here to login to the portal.

Amy Jo Caldwell, NP

Amy Jo Caldwell, NP is the Nurse Practitioner at the JPS school-based health clinic. Amy received her RN from Jamestown Community College,  her BSN from Daemen University, and her MSN FNP-BC from Gannon University.  Amy joined the JHS school-based clinic in 2024.  Outside of work, Amy enjoys camping, running, and traveling.  Amy is excited to be a part of the school based health center at Jamestown Public Schools and enjoys working with a great group of co-workers!
